New Patient Registration

Registering is simply a matter of completing your medical card or the completion of an NHS registration form.

All registering patients must complete a full medical with the Practice Nurse or HCA; this helps us to ensure all your records are up to date.

We do not discriminate or refuse registration on medical need, age, race, gender, social class, religion, sexual orientation, disability etc.

Re-Registering with the surgery

there may be occasions when patients wish to re-register with us eg have moved back to the area. These patients need to follow the same process as for New Patient Registration.

We will not re-register patients under the following conditions:

  • Have been removed or warned for violent/ threatening/ rude behavior
  • Have a history of unexplained multiple Did Not Attend ( DNA's) either at the surgery , or with secondary providers
  • Have a history of being un-contactable by the surgery on multiple occasions.
